Information about the employment compassionate to the dependant of deceased employee

The following information may please be furnished under RTI Act-2005
Reference to the Kolkata office order no.RMD/K/Pers/F-14/2009/321 dtd.11-02-2009, the eligible direct dependants of the deceased employees of Bolani Ores Mines applied for the aforesaid scheme and submitted their medical reports, death certificates and all other requisite documents. Then all the collection of documents sent to kolkata RMD head office for approval purpose,
1) it is requested, to please provide the name, Ticket No of the deceased employees whose file / order / documents got approved and state the date of dispatched to Bolani Mine Management? (make sure to provide information from the records of kolkata RMD office for this querry line. )
2) Please provide the unique Nos/reference nos. related to each of deceased employees file / order .
3) After dispatch from kolkata head office, in which date the document regarding the scheme reached to concerned office of Bolani Ores mines?
4) Please provide the names of direct dependants along with deceased relative in a table with heading Serial No., Name of deceased employee, T. No. of deceased employee, Direct dependants name with T.No, relation with the deceased employee. (make sure to provide information from the records of respective department of Bolani Ores Mines, Bolani-758037 (Odisha) for this querry line. )
5) Please provide the names of direct dependants of deceased employees sign & recieved the appointment order/letter from the personnel department, Bolani Ores mines.
6) Have the selected dependants valid requisite document to be prove them as direct dependant. please provide the xerox copies of documents submitted by the dependants.
7) Please provide duly signed declaration as follows at the end of the answer:
