RTI Requests

Sir,

I am Usha Goel retired Pvt School teacher located in Muzaffarnagar (U.P) & my EPFO account was in Merrut (U.P) after retirement i shifted Ludhina (Punjab) at the residence of my elder son. As Per requirement of ppo account no for transferring pension is required for that it is required to fill up 10D fourm duly signed by employer & self,  attached PAN Card & SBI Passbook Photocopy.All these document I have send to EPFO Merrrut (U.P)( 4 August 2012) .After that I have been send so many request to EPFO,Merrut by phone ,mail & epfi grievance management system always they have given same reply that they have forwarded all document after sign/seal to EPFO, Ludhina.Opposite to that it has been confirmed by Ludhina ,EPFO office concerned authority,but EPFO Office Merrut is not ready to aspect where is neglince occur due to which settlement becomes delay.

1. Copy of 10 D fourm after sign & seal by EPFO,Merrut.

2.Name of Person who is responsible for that negligence.

3.There is any provision of complaining & punishment to that employee by negligence of which Process becomes delay.

4.copy of the rule by which employee can be punished.

Copy of 10 D fourm(duplicate) signed by me & employer,

Copy of PAN card , SBI Passbook ,Letter by EPFO, Ludhina not received 10 D forum.
