There is no prescribed format as such.
you just have to make a postal order on the name of PIO , REGIONAL PROVIDENT FUND COMMISSIONER, payble at bangalore and send it .

I agree with Mr. Clean System 100%
Mr. Clean System has taken the pain of drafting and telling u everything on how to do things.
Now please take the pain of simply take the pain of cp-pasting the draft and other details to a MS word doc. put the postal order and mail it
    Moving this to Non-anonymity Category

    Hi Anand,
Thanks for your posting. My main intention of this RTI is not for knowing the status of grievance I filed.
I wanted to know what has happened to my application submitted to the PF office on 9th June 2011 for Partial Withdrawal of PF money. This is pending from their end from last 7 months. Till now there is no correspondence from them I tried to call them over phone many times but in vain.
I am from Hyderabad, this PF office is in Bangalore. So I can’t reach their office in person.
Many thanks to Clean System, now I got to know how to file an RTI from you, it is a great help to me.
I will go to a post office and take the PO and will send the application form.

Please answer to one last question, do I need to send this by only Normal Post or Registered post or can I send it through courier?

    Thanks,
    Satya

    • Anand - RTI Anonymous Team Member says:

My suggestion is send it thru SpeedPost , so that you have a tracking number or a Registered Post with Acknowledgement,
The main thing is to have a proper way to keep track of your application and also have proof that the document has actually been recieved by the concerned authority.
I would advice you to NOT use normal post, but use speed post or Registered with Acknowledgement

Thanks for the info, I will do it and post the results soon after I get a confirmation.

    Satya
