Sub-information according to RTI Act 2005

Sir

Provide me copy of documents provided by ultrasound clinics and received by your office relating to ultrasound center of Gharsana, in response to letter of your office Sn –Chikitsa/128.31 dated 1-2-12

1. As prescribed under Section 7 (1) of the RTI Act 2005, I request you to provide the information sought within 30 day period of receipt of the application. Any delay in good faith of providing the above information can be communicated in advance.
2. As per provision of RTI Act 2005, I request you to provide your details such as Name and Designation and Office address for further communication, if required.
3. As per provisions of RTI Act 2005, I request you to provide an acknowledgment for receiving the application, the acknowledgment would consist of date of receipt, file number/ application id and the details of the public information officer, OR you can send a photocopy of the application with countersign and sign and seal of the receiving office and other details of receipt.
4. As per RTI act I am attaching a IPO no of Rs. 10.00 for application fees. If you find that the sought information pertains to another PIO kindly transfer it within five days under section 6(3) .
