2.6 Name of agency conducting the work

2.8 Rate of tender for work on which contract awarded
2.9 Time schedule of payment made with date of payment & sum of amount paid
2.10 State if any payment is withheld. if so kindly state the reason for withholding payment
2.11 Copy of map for work if any.
2.12 Provide me copy of Documents /minute of meeting, on the basis of which decision to undertake developmental works were taken.
2.13 Give me list of officer/government servants/elected representative who supervised /inspected the work & given sanction for payments.
2.14 Provide me copy of work order register,labour register & master roll for work

    6) Name & Address of Contractors to whom the above said projects ( Para 3) were outsourced

    7) Certified copy of Progress report(s) of each of above said project ( Para 3)

    8) Copy of Tendering Process manual followed

    9) certified Copy of tenders received for all the above said projects ( Para 3 )

    10) certified Copy of Comparison report of tenders received

    11) certified copy of Minutes of Meeting of tender evaluation

    12) certified Copy of Payment advice/note of Advance payment for selected tender.

    13) Certified copy of documents/communications that shows the interest rates agreed for advance payment.

    14) Name, Designation and office address of the monitoring committee of each of the above said projects ( Para 3 )

    15) Name, Designation and office address of the supervisor(s) of each of the above said projects ( Para 3)

    16) Certified copy of all Purchase orders issued in connection with each of the above said project ( Para 3)

    17) Certified copy of all Work Orders issued in connection with each of the above said project ( Para 3)

    18) certified Copy of all agreements / contracts in connection with each of the above said project ( Para 3)

    19) Copy of work order register,labour register & master roll for work

    ** This will definitely diver the Resource of PA, therefore the person whose going to file this, should be prepared for inspection of records..

    Inspection of Records, is a bit tricky one..

    Inspection of Records:

    ** INSPECTION OF RECORDS is a diversion tactics, if you don’t go for inspection of records after invitation from PIO, then in the eyes of SIC/CIC it shows that you are not interested in getting information. Therefore it is suggested that you go for inspection and follow some tactics to handle things.

    1) Once you receive the invite for inspection of records, fix an appointment immediately.

    2) Take one person along with you

    3) Say that you want to inspect all the information that are listed in RTI

    4) If you are interested in getting photocopies of certain documents, you will have to make a note of it ( File number, page number and document name )

    5) Give it in writing that you need the photocopies of so and so pages.

    6) Ask him to communicate the details of additional fee to be paid to get those copies..[ Mostly it will be done instantaneously ]

    7) ** PIO or the person who is assisting you will be very polite most of the time, and may flatter you, and finally ask you to sign a declaration that the you are satisfied with inspection.
    Politely refuse it, say that you will send this only after you receive full information i.e. photocopies of those documents.
    In most of the second appeals this declaration of applicant is a hurdle for himself.

    ** Some cruel PIO may ask you to wait for half day. Dont get angry and impatient just record that in the inspection report.

    ** If they don’t behave well, don’t get angry maintain your composure and just file a complaint u/s 18, and pray for only disciplinary action.
