3. The amount of Fellowship and the Contingency expenses granted initially.
4. The present amount of Fellowship and Contingency expenses being paid to the applicant.
5. Name and address of husband if available on record.
6. The name of the university where the applicant has Ph.D. registration.
7. The name of the institution/centre where the applicant is studying/conducting Research.
8. The address where the Fellowship/Contingency payments/cheques are sent.
