Medical Facility Card ( DGEHS)

 

I applied on 10^th Dec,2011 for the DGEHS, Medical Facility Card after a gap of 24 years as I retired on 13.05.1988.

 

My ex-employer, Dhanpatmal Senior Secondary School , Roop Nagar Delhi-110007 forwarded the same to your office via letter No. DV PEN/CK/778 dt. 13.12.2011. Initially on retirement on 13.05.1988 my pension was fixed at Rs 1328 + RP Rs 240 total Rs 1568.

 

In my said letter dt. 10^th Dec,2011 , I had clearly stated that “ to contribute my subscription for the same ( for DGEHS facility) , by deduction from my pension , every month to be made as indicated in the scheme circular.

 

As per my basis pension I was supposed to contribute , Rs 30 per month as indicated in the pension range of Rs 1501-3000, as a DGEHS beneficiary, for 10 years in lump sum i.e Rs 3600 ( Three thousand six hundred only) mentioned in item no 3 of the said scheme circular , effective from 01.09.2003.

 

But inspite of my clear request to charge my contribution as a pensioner I was directed to deposit Rs 39000 ( Rs thirty nine thousand only) which was deposited to get my Medical facility card vide Challan No. 1593/15.12.11 on 15.11.2011 in State Bank of India , Old Secretariate Delhi-110054 , under protest, and by God’s grace my medical facility card was released in the 3^rd week of Feb 2012.

 

That I am 70 years old and only dependent, my wife is 64 years old. We do not know for how many more years we will survive.

 

I request your honour to please order for refund of extra deposit Rs 35400 ( Rupees Thirty five thousand four hundred ) to me. I was compelled to deposit Rs 39000 though actual amount to be deposited was only Rs 3600 at an early date to which favour I shall always be grateful.
