Sub: Information is requested  under the Right to Information Act-2005, towards study Leave granted to Dr. Manish Kumar Singh, S/o S B Singh, DLW Central Railway Hospital, North Eastern Railway, Varanasi.

I solemnly submit this application seeking certain definite and clear information and also request to provide the information on the following reference to the subject mentioned above under Right to Information Act-2005.

1)      Surce of appointment, Date of appointment, place of first appointment and designation  on appointment of the above said doctor?

2)      Period for which study leave is granted?

3)      Whether the above said doctor has satisfactorily completed a period of probation and has rendered not less than five years of regular continuous service including the period of probation under the government?

4)      Designation/Grade of the competent authority who has sanctioned/granted study leave to the above named doctor?

5)      Whether Head of the Medical Department certified that the study shall be valuable in increasing the efficiency  of such medical officer in the performance of his duties. If yes certified copy may please be enclose.?

6)      Whether leave salary is subject to furnishing of a certificate by the above doctor to the effect that he is not in receipt of any scholarship, stipend or remuneration in respect of any part –time employment . If submitted copy of same may please be enclose.?

7)      Whether said doctor has executed a bond as laid down in rule 4(4) of appendix V to study leave rules R I, undertaking to serve the government for a period of three years after the expiry of the leave, accordingly, a Railway servant having the option to retire will not be permitted to retire unless he serves  the government for three years after the return from the study leave. If submitted copy of same may please be enclose.?

8)      Whether  the General Manager has satisfied themselves that the required number of specialists are not already available and that the work can be managed without substitutes during the absence of above said doctor who have been granted study leave and that not more than Five doctors on each of the zonal Railways are granted study leave in a year?

9)      Name of the institute and course in which above said doctor has joined?

10)  No.of the doctor’s granted study leave during the year 2010 in  entire, North Eastern Railway, of Varnasi including DLW hospital may please be submitted in the below mentioned format:-
