Rules regarding passport issuance

A bank statement is accepted as an address proof while applying for passport .I want to know that what are the things that a bank statement should include. The main points i need information about are
1.whether the bank account must be more than one year old?
2. Is it mandatory that it must contain transactions every month compulsarily for the last twelve months ?
3. If the passport issuing officer create unneccesary harassment whom should we complain and how?
Sir,I don’t think asking for this information might cause any kind of problem. There is no need to file this RTI anonymousely
You don’t even need to file RTI for this. Answers are :
1.Yes.
2.Not every month but all the transaction in last 1 year.
3.Check i tout at passport.nic.in

My bank a/c was more than one year old but it was rejected on the ground that it contained only few transactions (5-6)in the whole year
.What should be done?I think there is no such rule that it should contain transactions of every month which i was told there.
File RTI asking the copy of the that rule!
