Please let us know the detailed services record of Shri Mukesh Kumar S/O Shri Dheeri Singh Yadav since his joining till date along with all the promotions he had during the tenure.

Please also let us know the if their are any discrepancies found in his promotions or in in his any of the appointment.

Please also let us know the detailed attendance record of him during his tenure in PNB Polytechnic in Mathura from year 2004-2005 till his tenure in the same institute. Please also provide furnished record of his medical leaves/ casual leaves and any other leaves he has taken for any reasons. Please attached the certificate he has produced to grant him any kind of leaves.

Please also present the detailed record of his expenses he has claimed during his tenure in PNB polytechnic including TA / DA and other allowances. Please also attach the copies of the certificates / tickets he has produced to claim those expenses.

Please also provide detailed record of his character and conduct during this period and mention if any DISCIPLINARY departmental action has been taken against him for his misconduct or any other reasons.

Please also provide detailed record for the permission given to him to attend the ME evening classes from year 2004-2005 and please also mention the detailed record of the leave he asked to attend the exams or classes.
