3. No of Childs alongwith there Date of Bitrth of Mrs. Kamla Rathi working here

as Supritendent.

4. Appointment procedure followed at the time of appointment of Mrs. Kamla

Rathi working here as Supritendent.

5. Educational Qualification at the time of joining of Mrs. Kamla Rathi working

here as Supritendent.

6. Present Qualification of Mrs. Kamla Rathi working here as Supritendent, &

Copy of  any permission taken for the same.

7. Is she is working somewhere else?, or member of any profit earning

organisation or NGO.

8. Total numbers of  Member of Staff working in Shishu Grah, & there working

hours.
