1.please give year wise account of activities undertaken under the disaster response scheme of the NALSA in the following format from the date of announcement of the scheme to today
a. year
b.state and district
c. type of disaster
d. period of disaster starting and ending [or on going as the case may be]
e. date of initiating the disaster response scheme
f. minutes of all meetings held to discuss , review and monitor the tasks undertaken under the disaster response shceme
g. total money allocated for the scheme , utilisation certificate/audited statement of the same and details of how much moeny spent for what purpose like educational material, meeting expenses remuneration etc etc
h. copies of educational material prepared if any
i. total number of claims received and action takne on them
j. other activities done under the scheme
k. name and address of the beneficiary under the scheme
