Sir
Under the provision of RTI act-2005 I request for following information pertaining to functioning of staff physician in GTB Hospital, Dilshad Garden Delhi.
1- Please provide the name of the current staff physician in GTB Hospital, Dilshad Garden, Delhi ?
2- Please provide the names of persons, who held held the office of the staff physicians in GTB Hospital between October 2001 to October 2011? Also provide their medical qualifications and the exact tenure they held this position
3- Please provide me the copy of the government order document, that describe the exact duties of a staff physician ?
4-Please provide me the copy of the hospital document, that shows how many hospital staff have been consulted by the staff physician between Octber 2006 to October 2011. This document should also include the Hospital Record Number and the names of the Hospital Staff.
5-Please provide me the exact number of Hospital Staff Member, their names and Hospital Record Numbers, that the staff physician has issued certificate of medical illness and fitness between October 2006 to October 2011
6-Please provide me the copy of the government order document, that describes what should be the educational qualification of a staff physician in GTB Hospital, Dilshad Garden, Delhi ? Also provide a the copy of the government order document, which indicates the policy of who appoints the Staff Physician in GTB Hospital
7- Please provide me the copy of the government order document, that explains the policy of how a staff physician can appoint a medical board for medical checkup of candidates appearing for checkup.
8> Please provide the copy of the government order document that explains who can become the member of the medical board.
9>- Please provide the copy of the government order document that explains the medical qualification needed to become physician in medical board?
10 > Please provide the copy of the government order document that shows all the medical boards constituted by Staff physician between October 2001 to October 2011
11> Please provide the copy of the document, which has the names and hospital registration numbers of those candidates appearing for medical checkup, whose medical checkup was done by staff physician himself between October 2006 to October 2011?
To satisfy provisions of RTI act-2005 I am submitting a Postal order of Rs-10 with Number______________, along with this another postal order of Rs-100 with number______________________ is also enclosd to bar expenses towards copy of documents.

Your request comes under the purview of RTI Act 2005.
But we feel that the request submitted by you, does not need you to be anonymous and hence we do not perceive any threat to you in filing this RTI Application.
To file this RTI Application you need to send postal order worth Rs. 35 (Rs 10 as fees for processing the application and Rs 25 as speed post charges of the reply) along with your RTI Application to the given address.

Thanks for your kind attention to my post. I agree that this RTI may not involve the extend of threat as described. But currently I am employed with this hospital and disclosure of my name may lead to harassment and difficult job conditions, which I do not want. There are many officials in this setup who are working in a manner which is non constitutional and this action may make them abusive towards me. It may also affect my Carrier prospects

the first thing the organization (that you would file RTI with) would do is to make a account over here and see who has filed this RTI. Having given your phone number, i am sure you would have made their task very much easier

Dear Mr. “RTIRTI” and Mr. “meradesh”
The focus of this organization is the help people file valid RTI requests, such that there is no harm to them, either physically or otherwise (career etc.). We will definitely ensure that this focus is never lost. Any frivolous requests, will basically mean that we have delayed helping these genuine RTI activists, who are in danger.
After reading the requestor’s comments/replies .. I would like to “trust and believe” that he is telling the truth, if he files this RTI himself, his career will be affected etc.
Now that Kamalji has rightly removed the phone number, Mr. RTIRTI please help us review “meradesh’s” RTI Application and draft it in a legally acceptable way. We all agree that the original request .. needs a lot of editting, so that the RTI application is not rejected.
Mr. RTIRTI : I completely understand your frustration and anger, bcos of about 80% frivolous requests, that this forum is being used by people and that is unfortunate.
But my sincere and humble request, let us focus on helping people and giving them the benefit of doubt, in this forum on RTI Anonymous. Let us refrain from using any wrong and negative words like “ridiculous”
