Dear RTI-A Team,

Please help to file this RTI application, I am unable to do it (I learned that its tedious to file from abroad) from Hong Kong and thus need your help to file my 2nd RTI, which will hopefully bring better healthcare facilities in my hometown. Appreciate if you can review, and improve the application before submission ( also I am not sure if PIO mentioned below is the right one but this is what I could find).

Dear Sir,

Under the RTI, here are the particulars of information required:

PHC, Jamania

1. PHC, Jamania – provide the total area on which PHC is build, number buildings and their usage?

2. Year of Construction completion, service start date and Cost of Construction of PHC in Jamania?

3. Details of all the facilities available (Ward, beds, machinery, equipment, test facilities etc) in PHC, Jamania, including permanent and temporary staff?

4. Per month patient visit (as per the OPD record) of last one year and total fee collection per month?

5. What are all the medicine made available to the hospital for free distribution in last one year (from the day of receipt of this application)?

6. List medicines and their quantity distributed free in last one year  (from the day of receipt of this application)?

7. Contact details of responsible person to purchase, manage the inventory of medicine to be distributed free?

10. How many times the PHC Building has been renovated in last 10 years, each time how much expense occurred, with detailed break down of cost/expense for different purposes? ?

11. How much is monthly average expense to run this PHC, how much is the revenue generated per month from this PHC, provide the information for last 12 months ( from the day of receipt of this application)?

CHC, Baruin (Jamania)

1. CHC, Baruin (Jamania) – provide the total area on which CHC is build, number buildings and their usage?

2. Year of Construction completion, service start date and Cost of Construction of CHC in Baruin?

3.  (a)Details of all the facilities available (Ward, beds, machinery, equipment, test facilities etc) in CHC, Baruin (Jamania) , including permanent and temporary staff?

(b) Also provide the usage report – monthly of all the equipment of test/pathology tests?

4. Per month patient visit (as per the OPD record) of last one year and total fee collection per month?

5. Provide the Social, Economic, and Administrative reasons (if any) of establishing a CHC in a village which is not well connected to nearby areas?

6. What are all the other/any reasons of starting this Facility in Village Baruin and not having it started in Jamania which is basically Tehsil Headoffice and center location for nearby areas..?

7. What are all the medicine made available to the hospital for free distribution in last one year (from the day of receipt of this application)?

8. List medicines and their quantity distributed free in last one year  (from the day of receipt of this application)?

9. Contact details of responsible person to purchase, manage the inventory of medicine to be distributed free?

10. How many times the PHC Building has been renovated from the year it was built, each time how much expense occurred, with detailed break down of cost/expense for different purposes?
