There is scam in awarding the project for running of Emergency Response Services” popularly known as “108 Ambulance Service project in Rajasthan
Please provide me Information relating to “Emergency Response Services” popularly known as “108 Ambulance Service project in Rajasthan”
1. Please provide me copy of applications with attached documents which were received in response to notice for INVITING REQUEST FOR PROPOSAL (RFP).
2. Provide me details of minutes of pre-Bid conferences with all the bidders.
3. Provide me details of evaluations reports with all file notings of Technical Part of proposals .
4. Provide me copy of document as under for all the Bidders
1) Covering Letter along with the original power of attorney authorising the authorised signatory to act on behalf of the firm/consortium.
2) Brief profile of the bidder.
3) Details of Bidder as specified in the format given in RFP document.
4) Documents/certificates/evidence of fulfilling the eligibility criteria including Audited financial statements for the last 3 (three) financial years i.e., 2006-07, 2007-08 & 2008-09.
5) Related certificates of experience.
6) Other documents which the bidder supplied in support of his proposal.
5. Provide me the details of bidder to whom project was awarded with reason with copy of all the documents regarding this.
6. Provide copy of agreement between NRHM & bidder.
7. provide me details of money reimbursed to Bidder month wise with details of different heads of reimbursements. Since awarding of contracts.
7. copy of log book of all the ambulances since awarding of contracts.
Provide information in compact disc.
I suggest you not to ask for compact disc..
later you will regret your decision..
Its better to get hard-copies attested by competent authority with “Furnished Under RTI” seal on it..

If logbook entries are fed into computers it would just a click of button else there is chance that …
This may be denied on the grounds that it would disproportionately divert the resource of PA..
Can this be made look much simpler without defeating the cause..

Log book of is very important information as false bills are reimbursed from the government without actually doing the tour
false bill of medicines are reimbursed even in the absence of medical officer
terms of contract also changed from time to time to suit the bidder .even after awarding the contract( even you can incorporate his also in RTI)
Contract is awarded to company named jigista ( company of son of P chidambram & Viylar Ravi)
There are rumors that there is misappropriation of fund at the tune of of Rs. 3000 million in this contract & refurbishments
